6 Steps to apply Support at Home (SaH)

 

- 65 years old or older,

- Australia citizen or permanent resident,

- needs help with many everyday tasks at home, or has more complex or intensive care needs due to old age and medical conditions. 

You can call My Aged Care 1800 200 422 and answer a few simple questions. If you meet the requirements, an assessor will arrange to meet you in person, usually at your own home.

You can also choose to apply online. Please have your Medicare card ready when applying. The process takes approximately 10–15 minutes.

My Aged Care Assessments Team will contact you to arrange a time to visit. Assessments are done in person, at your own home. You don’t need to go anywhere. The assessor will have a copy of the information you gave to the My Aged Care. This gives them an idea of what support you might need to help you return to, or keep the level of independence needed to manage your day-to-day life at home.

After your assessment, you will receive a letter outlining the assessment outcome. It will confirm whether you are eligible for Support at Home (SaH), the level of funding you’ve been approved for (if eligible), the reasons and evidence supporting the decision, and include a copy of your support plan developed during the assessment.

Once you are approved for funding, there may be a waiting period before it is assigned to you, depending on the national priority system. In the meantime, there are steps you can take to prepare, so you’ll be ready to arrange your care services once your funding becomes available.

You will then receive a second letter with a referral code, confirming that you have been assigned Support at Home funding. Contact us on to get started with your support services.

You will have 56 days from the date of the letter to enter into a Service Agreement with a reliable and quality-approved Support at Home provider, like us. If this does not occur within that timeframe, your funding will expire and be reallocated to the next person on the national priority system.

Once you have chosen your preferred provider, if you have chosen us - Active Community Group, a member of our Care Team will personally visit and offer you a Home Care Agreement, then discuss & design your personalised care plan and budget plan together with you and your family / carer at your own home, before you begin to receive services and support at home.
