About Us

Established in 2012, we are a trusted, government-approved provider of Support at Home (SaH) and the Commonwealth Home Support Programme (CHSP).

Our mission is to deliver high-quality home care and wellbeing services that empower older Australians to live independently and affordably. Serving the metropolitan communities of Melbourne - Victoria, Adelaide - South Australia, and Sydney - New South Wales.  we are dedicated to helping you navigate the aged care system and find the support that best fits your lifestyle.

 

Support at Home (SaH)

The Support at Home (SaH) program is a government-funded initiative designed to help individuals aged 65 and over remain independent in their own homes for longer.

These packages offer the flexibility to choose the specific services that matter most to you. In-home care is a powerful alternative for those who wish to stay in their familiar surroundings while receiving the extra support needed to remain active and engaged in the community.

Depending on your allocated level of support, our services are designed to:

 

  • Maintain Your Wellbeing: Professional care that keeps you healthy and independent.
  • Ensure Your Safety: Practical assistance to make your home a safe environment.
  • Stay Connected: Support that keeps you involved with your local community.

 

 

Commonwealth Home Support Programme (CHSP)

We are the official CHSP provider, to deliver essential, entry-level support to seniors who need a helping hand to manage at home.

The program is built on a "restorative" philosophy—working with you rather than just doing things for you. By providing targeted assistance, we help a wide range of people maintain their autonomy with just a little extra help.

Most CHSP clients benefit from one or two key services to stay independent. Our available supports include:

 

  • Domestic Assistance: Reliable help with cleaning and tasks around the house.
  • Transport Services: Ensuring you get to appointments and community events safely and on time.

 

Aged Care Volunteer Visitors Scheme (ACVVS)

Connecting Hearts, Building Community. 

Companionship is the cornerstone of healthy aging. Funded by the Australian Government, the Aged Care Volunteer Visitors Scheme (ACVVS) plays a vital role in providing social connection for older Australians living in aged care homes or at home while receiving (or waiting for) a Support at Home. 

In partnership with AuSCC, Active Community Group is officially authorized to facilitate these meaningful connections. We specialize in providing free visitation services for seniors from Culturally and Linguistically Diverse (CALD) backgrounds. Our goal is to ensure that every senior feels seen, heard, and connected, in the language they know best.

  • A Gateway to Support: A bridge to the wider community for those living at home or in residential care.
