Premium Management - Level 2-4

Introducing ACG’s newest initiative, our 35% Premium Management option! Available for HCP clients with Level 2 or higher, experience exceptional care and support from a dedicated Care Manager. Clients under this management option will have access to our bonus services.

Commonwealth Home Support Programme - CHSP

Active Community Group provides domestic assistance and transport services to clients through CHSP (Commonwealth Home Support Programme) funding. These services aim to support elderly individuals in maintaining their independence and quality of life at home and within the community.

Our home assistants help with essential household tasks, such as cleaning service. Additionally, we offer reliable transport services to assist clients with medical appointments, shopping, and social activities, ensuring they remain connected and engaged in their daily lives.
